GP Practice based Clinical Pharmacist

The Waterside Primary Care Network are a forward thinking and dynamic group of 3 practices with 43,000 patients. We are seeking a like-minded practice pharmacist, preferably with previous experience of working in primary care, to develop and manage medicines optimisation services within our practice.
This is an exciting opportunity to develop a new role within this newly formed Primary Care Network. The Clinical Pharmacist will work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions and advice for those on multiple medicines. The role is pivotal to improving the quality of care and ensuring patient safety.
Having a clinical pharmacist in post means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.
The successful candidate will have a post-graduate diploma or higher degree in Clinical Pharmacy, and will be an independent prescriber or working towards this.
Part-Time and Full-Time considered.
